I authorize Baby Logic 3D/4D Ultrasound Studio LLC (hereinafter referred to as Baby Logic) to perform an ultrasound on me and my baby. I elect to have this procedure, and I understand that it's purpose is not diagnostic in nature, as in the procedure is for the purpose of obtaining a 2D, 3D, or 4D view of my baby in the womb over a period of time scheduled by appointment. I am aware that this service is not covered by any insurance providers. I have established prenatal care with a provider of my choice. These ultrasounds are in no way intended to replace a diagnostic study. I acknowledge that, during this session, an optimal view is at the discretion of Baby Logic staff, and that this may not always be possible at the time of the appointment, extent of what is seen does depend on the position and size of baby. The image quality can vary, and the fee is nonrefundable. Measurements will not be taken, and anatomy will not be evaluated, which means that there is no physician reviewing images. STRICTLY for enjoyment purposes ONLY! I also understand that Baby Logic owns the rights to any photos, videos or images produced from this session. In consideration of services rendered, the receipt and sufficiency of which are acknowledged, I agree to release and hold harmless Baby Logic, its agents, employees and affiliates from any and all claims or causes for action for injury, harm, loss, damage or other liability which result from or are alleged to result from this ultrasound.
